HF-ACT 

Application Requirements:

Housing First – Assertive Community Treatment Team requires applicants to have a documented severe mental illness as well as be chronically homeless, for a year or more.

  • Mental Health Diagnosis
  • Referral from a Medical Professional
  • Copy of Government Issue ID
  • Copy of Medicaid Card
  • Copy of Social Security Card

ICM- RC/ ICM-SA 

Application Requirements:

Intensive Case Management – Recovery Coordination & 

Intensive Case Management – Substance Abuse Teams require applicants to have a substance abuse or mental health diagnosis.

  • Completed Application for Services
  • DSM-5 (Adult Self Rated Symptom Measure)
  • ACE (Adverse Childhood Experience)
  • BAM (Brief Addiction Monitor)
  • Copy of Government Issue ID
  • Copy of Medicaid Card
  • Copy of Social Security Card

Any diagnoses submitted would be helpful and expedite the process, but is not mandatory.

PATH

Projects for Assistance in Transitioning from Homelessness

  • Completed Application for Services
  • Copy of Government Issue ID
  • Copy of Medicaid Card
