Projects for Assistance in Transitioning from Homelessness

The housing program is an outreach program that assists homeless individuals with mental illnesses living outdoors or in places not meant for human habitation, to access suitable services or housing. Our housing program conducts outreach in the Anchorage, AK community meeting consumers where they are.

 

Funded by a Federal Grant through SAMSHA.

Successfully housed 25 individuals.

The Housing Program (PATH) has been operating with CHOICES since July, 2018.

Evidenced-based, culturally competent and strengths-based outreach techniques are used to engage individuals who are homeless and are experiencing mental illness.

The Housing Program aims to re-integrate consumers with financial, medical, employment and housing services by connecting them with services either internally or externally.

 

Our Housing Program can occasionally help with resources towards housing, as well as emergency items needed for survival such as hand-warmers, water, snacks, and warm clothing.

If you are an individual who could utilize assistance from the housing program, please fill out the referral form and
email, fax, or turn in a hard copy.
